GENERAL PRICE LIST
Items A through J are the total of fees required for a
complete funeral service.
You may elect not to use any item with the exception of
Item A.
PROFESSIONAL SERVICES
A. MINIMUM SERVICES OF THE
FUNERAL DIRECTOR &

This fee will be added to the total
cost of the funeral arrangements you select. (Fee already
included for direct cremation, ship-in and ship-out packages.)
Includes 24 hour staff and facilities to respond to initial requests
for service, arrangements conference with responsible party to
determine services desired, and coordination of arrangements with
cemetery, crematory and/or airline for shipment. Includes
coordination of funeral participants, supervision and attendants,
arrangement of floral and other memorial offerings, and supervision of
music during service (if necessary).
B. EMBALMING
&
NECESSARY PREPARATION OF

Except in certain cases, embalming
is not required by law. Embalming may be necessary if you select
certain arrangements such as a funeral with a viewing.
C. DEATH CERTIFICATE &

necessary signature(s), then delivery to proper registrar for state
filing.
USE
OF FACILITIES

This fee may be substituted for the additional
time, personnel and equipment required for church visitation when held
just immediately prior to funeral ceremony.
E. USE OF
FACILITIES FOR

Includes use of music facilities. This fee may be
substituted for the additional time, personnel and equipment required
for church ceremony. Additional fees will apply for evening
service. If graveside only ceremony is held, this fee shall be
$195.00.

Removal exceeding thirty (30) mile radius of
funeral home will be charged on a per mile basis plus base fee of
$275.00. This fee does not include reimbursement for any transfer
expense incurred by Medical Examiner or other parties for examination,
to perform autopsy, etc.
G. USE OF FUNERAL

To cemetery, crematory or airport. Thirty mile radius of
funeral home, additional charge on per mile basis. For an evening Church
service with Graveside service the following day, the fee will be added
twice.
H. LIMOUSINE
SERVICE - Per

By funeral home's limousines. Ten mile radius of
funeral home at no additional charge. Additional limousines or other
livery equipment available on a rental charge basis.

The
goods & services shown above are those we can provide to our
customers. You may choose only the items you desire.
However, any funeral arrangements you select will include a charge for
our services. If legal or other requirements mean you must buy
any item you did not specify or ask for, we will explain why in writing
on the statement we provide describing the funeral goods and services
selected. This list does not include prices for certain items
that you may ask us to buy for you, such as cemetery or crematory
services, flowers, newspaper notices or a motorcycle escort. The
prices for those items will be itemized on the statement of funeral
goods and services you selected. In a few cases, we charge for
our services in buying these items. Caskets offered range in price from
$895.00 to $9,690.00. These prices are outlined on the
Casket Price List. A copy of our General Price List is available
to those requesting such in person.
CREMATION
A. DIRECT

Includes removal of
remains (25
mile radius of funeral home, additional mileage $1.50 per mile, one
way),
transportation to crematory, container for body as required by law,
obtaining
signed death certificate and filing it with the state, cremation permit
and all
necessary authorization, cremation of body and minimum services of
staff.
B. CREMATION WITH
MEMORIAL
SERVICE

all of the direct
cremation services listed above, plus use of our facilities and use of

staff for a memorial service or memorial visitation equipment, printed

folders, register book, acknowledgement cards and use of our music

Complete
traditional services with
body present at ceremony for viewing.
Solid Oak Rental Casket is included as well as cremation
following. This is a complete charge including
embalming, cosmetology and full preparation of the body.

of the above include a
temporary container for the cremains.

outer burial containers other than those made by the manufacturer. The

warranties, expressed or implied, granted in connection with goods sold
with a funeral service are the expressed written warranties, if any,
extended by
the manufacturer thereof. No other warranties and no warranties of
merchandising
fitness for a particular casket are extended to the purchaser.
